TrueBearing101 Disclosure Agreement
This service agreement is effective upon completion between the following parties: TrueBearing Academy (TBA) of 2910 E Madison St, Suite 209, Seattle, WA 98112, and [ClientName].
During the term of this Agreement, TBA shall perform/provide the following services:
- Four individual mentoring sessions:
- Session 1 (30 mins): Introduction, goals, and expectations
- Session 2 (60 mins): Conducting and interpreting the TheraQ; setting your first protocol
- Sessions 3 & 4 (45 mins each): Guidance with additional TheraQ assessments, protocol selection, and results review
- Complimentary access to the next two TrueBearing Grand Rounds sessions
- Unlimited access to current TB101-related videos and tutorials for 24 months; access may be extended upon request
Cancellation Policy
Please remember to cancel or reschedule your mentoring sessions at least 24 hours in advance. The first late-cancelled or missed mentoring session may be rescheduled. Subsequent late-cancelled or missed mentoring sessions will be counted as a session.
